PRIVACY NOTICE STATEMENT

We are authorized to collect personally identifiable information (PII) from you by applicable federal and state laws. Any PII we collect is used to create and manage your account, and to communicate with you about your account. PII is also used to help you apply for financial help and/or enrolling in a Marketplace qualified health plan (QHP). Protecting your privacy is very important to us. This privacy policy describes what information we collect, why we collect it, and what we do with it.

If you choose to give us PII, we may share this information with Centers for Medicare and Medicaid Services (“CMS”), and will be maintained in a federal System of Records. Your PII will also be shared with other related entities. See HOW INFORMATION MAY BE SHARED section for detail.   • If you are a licensed insurance agent using benefitalign®’s agent platform, the designated administrator of your account, if you have an individual subaccount, regarding the Services by individual subaccounts;
